Claude (agent-headless-import) 87f6c96b53 docs(import): CSV text read by role, and the headless import refusals (improvise-8jk6.4, improvise-8jk6.5, improvise-8jk6.6)

context/code/formula-and-io.md gains the argument for text-first CSV and for
the two refusals, and puts the partial-skip exit code to the user.
context/code/cli.md §13 states the headless contract. Evidence entries are
added for the new tests, with notes on the three parser entries whose
premise moved.
